Enrollment
Registration is open Monday thru Friday 8:00 a.m. to 4:00 p.m. at 1230 Palmyra Rd. SW Warren or via the clicking one of the buttons below. Applications can be completed by appointment at all outlying Head Start centers during the school year. Appointments are preferred but walk-ins accepted.
Be sure you have the following items

Proof of Birth - Birth certificate, Crib Card, Hospital Birth Record

Shot Record - Must include the MMR vaccine
​
Proof of Health Insurance - for the child if the child has medical insurance
​
Proof of Income - All household income for the last 30 days or the last 12 months from the date of application. Previous year's tax information is accepted.
​
Photo ID - Must be a state issued photo identification for parent(s) or legal guardian of the child.
​
Social Security Cards - or the number for all family members.
* Potty Trained is not required!
OTHER DOCUMENTS NEEDED
Physical Exam - Children are required to have a physical exam within 30 days of their first day of school.

Dental Exam - Children are required to have a dental exam within the last 12 months.
